Services:

A social services program to provide information and advocacy to the community and to the consumers of long term care, in nursing homes, adult care homes and to consumers of home care services, and to investigate and resolve residents rights violations and seek resolutions. Volunteer opportunities are always available. Speakers are also available.

Any consumer receiving long term care

Assisted Living to older and/or disabled adults, seeking to retain their independence while receiving services they need. Services tailored to reflect the needs and choices of each resident.

 

Eligibility:

Private pay, although, some long-term care insurances provide assistance.

Cost of Services:

Levels of care and size of apartment vary. Price range: $1565.00 to $2730.00. Changes may occur on a monthly basis, including: rent, basic utilities, routine maintenance, housekeeping. Three meals are included in monthly rate. Pets are welcome.

            If resident is admitted to facility, cost is $88/day. No fee for information             services.

             - To provide assessment and referral services in the community.

            - To provide an alternative to nursing home care.

            - To provide home care services to persons who are income-eligible with

              appropriate health care needs.

            - To provide assessment of “level of care” required for Medicaid recipients.
